Collaborating On Novel Post-Acute Pathways: A Value-Based Performance Improvement Initiative

In order to excel at value based care, Orthopedic groups must implement new pathways and innovations to optimize TJR bundles. It is well known that the coordination of post acute care can drive cost savings, primarily when shifting away from the overutilization of home health. Do these innovations reduce costs, maintain quality, and improve patient satisfaction? What is required to make performance improvement successful? This presentation will walk through the implementation of a novel outpatient PT at Home platform when adopted by one of the largest Orthopedic specialty centers in the country. We’ll present the keys to successful adoption, in addition to real world cost-analysis and outcomes.

Biography

Sopida Andronaco joined Hoag Orthopedic Institute in 2012 and is the Director of Performance Improvement and Clinical Outcomes. In her current role she aims to improve and sustain superior clinical outcomes, while minimizing costs. Ms. Andronaco oversees performance improvement initiatives, patient satisfaction, patient outcomes, infection prevention and Centers for Medicare and Medicaid Services quality reporting. She facilitates HOI’s multidisciplinary Performance Improvement Committee to spearhead an innovative model for value-driven healthcare.

Biography

Palak Shah, passionate healthcare leader and physical therapist for 15+ years with orthopedic specialist board certification. She leads clinical vision & strategy through operational excellence. She excels in data & technology driven approaches for new clinical products, clinical operations and program design.

Palak started as an outpatient PT and obtained Orthopedic specialization from the ABPTS in 2014. She spent 4 years at Stanford Hospital as a senior physical therapist and co-lectured at the physical therapy residency program. Palak is now a co-founder and Head of Clinical Services at Luna, which is a leading provider of in-home, outpatient physical therapy services.
